How to apply for Long-Term Care

Making the choice to leave the familiar surroundings of home in favour of receiving 24-hour health care can be a difficult decision.  It is equally difficult for extended family members who are concerned about their loved ones.

If you are at the point of making this decision in your own life, we are here to help.  All admissions to a long-term care home are handled through the South West LHIN (contact 519-371-2112)

You may wish to view our Home to see if it is a right fit for you or your loved one.  Simply contact us by telephone at 519-793-3433 or Email  We would be happy to arrange a tour of our Home and answer any questions you may have.

Like any major life change there is a time of transition when moving into a long-term care home.  At Golden Dawn, our caring staff help to make the transition an easier one for both our residents and their family members.

What Will it Cost?

Golden Dawn is a publicly funded, not-for-profit organization, licensed and regulated by the Ministry of Health and Long-Term Care in Ontario.

While the Ministry of Health and Long-Term Care pays for your care, you are responsible for the co-payment accommodation costs.  These costs are standard across Ontario and are reviewed by the Ministry of Health and Long-Term Care on an annual basis and are generally adjusted on July 1st, based on inflation.
If you are eligible for placement in a long-term care home you will not be refused because of an inability to pay.  Rate reduction subsidies are available for those without enough annual income, and living in basic accommodation.  The subsidy is not available for those who request a semi-private or private room.

Our Administrative Office personnel will be happy to provide you with information about your eligibility and how you can apply for a rate reduction subsidy.  For our current accommodation rates
